What To Send

What I Need From You:
1. Maxillary and mandibular alginate impresions.
2. A wax bite for accurate occlusion of the finished models.
3. A completed lab slip for each set of impressions.
4. Please enclose as many sets of impressions per box as possible.

Packing and Shipping
1. Wrap impressions in a damp paper towel. Place in a ziplock bag and seal tightly. Write patient’s initials on bag with marker and attach completed lab slip.
2. Place sealed impressions in the shipping box with cushioning material (newspaper, etc.) and tape the lid shut.

3. Affix the mailing label to the box top.
4. Fill in your return address on the mailing label.
5. Obliterate any other addresses and barcodes on the outside of the parcel.
6. Give the box to your postal carrier.

PLEASE NO STANDING WATER IN BAGS!
Paper towels used to wrap alginate impressions should be moist, not dripping wet. Be sure bags are sealed tightly. Leaking or wet boxes require HAZMAT procedures at the post office that may delay shipment.

Turnaround Time
You will receive your finished models approximately two weeks from the day the impressions leave your office. Please indicate due date on lab slip. Call for ABO model turnaround times.
